Advise us about a Directory data change

When an authorised library service staff member makes a change to one of the data records used for the Directory of public library services, the system automatically advises State Library staff that a change has been made.

This form can be used to let State Library staff know the specific nature of the change/s you have made to a record. Note: it is not mandatory to send SLQ this information.

All fields are *mandatory.
